Memorial Photo Tag Purchase Form
The City of Oak Hill has completed the fencing for the Dr. Hamilton Dog Park and K-9 Memorial on Central Avenue (corner of Central Ave. and School Street). The K-9 Memorial section contains a mausoleum for the remembrance of local K-9 officers that have passed away. However, there is an opportunity for everyone to memorialize their own dog(s) with photo tags that will be hung on the dog park fencing.
To obtain a tag, fill out the form today and remember that special dog (or several) in your life! We can print the name and dates on the front or back of the tags. Let us know below.
$25 Price Per Tag
Include a close-up photo of your dog as the tags are two inches in diameter.
